Accessing Institutional Personal Information at U91探花
The collection, use, or disclosure of personal information at the University of 91探花 (U91探花) must comply with Alberta鈥檚 Protection of Privacy Act (POPA), and Health Information Act (HIA). Personal information means any recorded information about an identifiable individual, including data derived from personal information or any de-identified or anonymized non-personal data that could be used to identify any individual either alone or in combination with other available data. Records containing personal information may only be used or disclosed in accordance with applicable legislation and U91探花鈥檚 Privacy Policy and Privacy Management Program.
Individuals may request access to their own personal information in the custody or control of U91探花 by making a formal access to information request under ATIA. Please complete the Request to Access Information Form and submit it to accessandprivacy@ucalgary.ca.
Researchers may request access to institutional data in the custody or control of U91探花 by making a request under POPA. Please complete the Request to Access Institutional Personal Information Form and submit it to accessandprivacy@ucalgary.ca.
Requests for access to institutional personal information for research purposes require ethics approval and approval from the relevant Information Steward. Ethics applications may be submitted to both the Access and Privacy Office and the applicable Research Ethics Board concurrently to support timely review and reduce potential delays. Access will not be provisioned until all required research ethics approvals have been obtained.
If the request is approved, the requestor may be required to sign an information sharing agreement prior to obtaining access with approved conditions regarding security and confidentiality. Note that a fee may be charged to de-identify the requested information. Provisioning of the data will be dependent upon availability of resources, institutional priorities and timelines.

U91探花 faculty or staff may request access to institutional data held by another faculty or administrative unit for use in an institutional practice, program, project or service. Please complete the Request to Access Institutional Personal Information Form and submit it to accessandprivacy@ucalgary.ca.
Requests for access to institutional personal information for institutional purposes require approval from the relevant Information Steward. There may also be a requirement to conduct a privacy impact assessment (PIA) where required under POPA.
